Thanks.  I downloaded updated support tools, and the updated ktpass does 
recognize /MITRealmName.

For what it's worth (and in case anybody goes searching for this later), 
using Windows Update will happily update the OS service packs, but did not 
update the Support Tools.  I had to manually download the cab and msi.
